29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about March 8, 2023, employees installing shingles on a new residential home, used extension ladders to access the roof that were not erected three (3) feet above the upper landing. This exposed employees to a fall hazard of approximately 11 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about March 8, 2023, employees conducting roofing work on an new residential home with an 8:12 pitch were not using fall protection while installing new roof. This exposed employees to a fall hazard of approximately 11 feet to the ground below. E.V. General Construction Inc. dba EV Construction & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11) , which was contained in OSHA inspection number 1575776, citation number 2, item number 1 and was affirmed as a final order on August 30, 2022, with respect to a workplace located at 745 S Buffalo Grove Road in Buffalo Grove, IL 60089. E.V. General Construction Inc. dba EV Construction & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) , which was contained in OSHA inspection number 1326420, citation number 1, item number 1 and was affirmed as a final order on October 30, 2020, with respect to a workplace located at S88 W12931 Ballard Court in Muskego, WI 53150.
