29 CFR  1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  a) Hyde Avenue Lot #245, Auburn Meadows, Smyrna, DE 19977: On or about March 10, 2023, employees worked on site to conduct framing operations and were exposed to multiple hazards including but not limited to falls, burns, and struck by hazards which had not been identified due to a lack of programs that provided for frequent and regular inspections made by a competent person.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Hyde Avenue Lot #245, Auburn Meadows, Smyrna, DE 19977: On or about March 10, 2023, employees exposed to a fall hazards when working approximately 15 feet above the next lower level while framing a residential structure without fall protection. Employees were walking and working on the timber framed structure and used the interior structure to climb the surface instead of using ladders.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Lot #245 Hyde Avenue, Auburn Meadows, Smyrna, DE 19977: On or about March 10, 2023, employees were exposed to fall hazards while walking and working approximately 15 feet above lower level without the use of any fall protection training during a rainstorm. It was determined the employees were not provided training to recognize fall hands and how to minimize them while conducting framing activities on site.
