29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: a) At 1548 Trentwood Place NE, Brookhaven, GA 30319; On or about March 14, 2023, and at times prior, the employer exposed employees to amputation, in that the guarding of a Makita 5007F 120V SN:1961445 circular saw was blocked by a piece of wood, while performing siding work.

29 CFR 1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) At 1548 Trentwood Place NE, Brookhaven, GA 30319; On or about March 14, 2023, and at times prior, the employer exposed employees to falls of about 19 feet to a lower level, in that personal fall arrest systems or guardrail systems were not provided to employees while performing siding work on a pump jack scaffold.
