29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 16, 2023, employees were in use of lathes that did not have guards installed. This exposed employees to caught in hazards. a. The Takamaz XD 10 I Lathe did not have a guard installed. b. The Okuma LB 3000 X II did not have the guard installed.

29 CFR 1910.212(a)(3)(iv)(e): One or more methods of machine guarding were not provided to protect the operator and other employees in the roll milling area of operation (front and back) from hazards such as those created at the point of operation, and ingoing nip points. On or about March 16, 2023, employees were in use of the Burke milling machine that did not have the proper guards installed. This exposed the employees to a caught in hazard.
