29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE):   On or about March 28, 2023, the employer failed to assess workplace hazards that necessitated the use of PPE when employees were exposed to splash and irritating mist hazards while pouring and mixing injurious materials that cause serious damage to the eyes and skin. A determination of the proper protection was not made for chemicals handled during the label generation process that included, but were not limited to, the use of Category 1 UV LED Flexo Ink and Category 2A Liquid Plastic resin in the following areas:  a) Digital Production b) Pre-Press Area

29 CFR 1910.133(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when they were exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Digital Production:  On or about March 27, 2023, and sometime thereafter, the employer did not ensure that each affected employee used the appropriate eye or face protection when exposed to injurious liquids that splashed while handling corrosive products that included, but were not limited to, a UV LED Flexo Ink (Product Code: 6005306968) that was used without a suitable facility for quick drenching or flushing of the eyes available within the immediate vicinity of the work operation for emergency use.  b) Pre-Press Area:  On or about March 27, 2023, and sometime thereafter, employees were exposed to injurious liquids that splashed while handling corrosive products including, but not limited to a corrosive 11776 HC Cream mix that was used without a suitable facility for quick drenching or flushing of the eyes available within the immediate vicinity of the work operation for emergency use.
