On or about April 11, 2023, at the above addressed jobsite, the employer did not ensure that employee(s) who operate forklifts were properly trained, evaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.

29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:(a)  On or about April 11, 2023, at the above addressed jobsite, employees working on the ground, below carpentry activities, were not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On or about April 11, 2023, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.  The JZ Custom Framing LLC was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 1622118, citation number 1, item number 2 and was affirmed as a final order on February 3, 2023, with respect to a workplace located at 20674 Little Transverse Bay Drive Frankfort, IL 60423.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about April 11,  2023, at the above addressed jobsite, employees performing residential carpentry operations at approximately fifteen (15) feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  The JZ Custom Framing LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1622118, citation number 1 , item number 3 and was affirmed as a final order on February 3, 2023, with respect to a workplace located at 20674 Little Transverse Bay Drive Frankfort, IL 60423.
