29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): In the Main Plant, employee(s) engaged in material handling activities while operating four forklifts were exposed to struck by and crush by hazards in that the wheels/tires on three forklifts were found to be in disrepair with several cracks, fissures, and/or pieces missing causing the powered industrial trucks the inability to carry a stable load, and in one forklift the horn was inoperable causing the powered industrial truck the inability to notify of movement. The forklifts with tires in disrepair were as follows: 1. Make: Crown #22; Model: CGC20SC-9; Serial No. FGA1C-1450-00420; ID#: A256015, located in the Ground Beef area. 2. Make: CAT #4; Model: GP20CN; Serial No.: A134U2187; ID#: C99-24981; located in the ALVEY. 3. Make: CAT #99; Model: GP20CN; Serial No.: A134A50138; ID#: Not Legible; located in the Freezer Loadout area. The forklift with the horn inoperable is as follows: 1. Make: CAT #20; Model: 2ET4000; Serial No.: FN541786; ID#: EO583; located in the Freezer Loadout area.
