The forklift was used in an unsafe manner when dumping rendering material with a self dumping hopper.  The forklift when coming to a sudden stop with the hopper raised in a dumping motion resulted in the forklift tipping forward with the mast resting on the dumpster.

The tread on all four tires on the Toyota LP forklift were completely worn causing the powered industrial truck to lose traction in a slippery environment,  2. The horn on the Toyota LP forklift was inoperable causing the powered industrial truck the inability to notify of movement.  3. The lights on the Toyota LP forklift were inoperable causing the powered industrial truck to have insufficient visibility.  National Beef  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(p)(1), which was contained in OSHA inspection number 1662918, citation number 1, item number 1 and was affirmed as a final order on 07-11-2023, with respect to a workplace located at 1501 E 8th St. Liberal, KS 67905.
