29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields: a) Miscellaneous Line - On or about April 11, 2023, and at times prior thereto, employees who were in the area of the Miscellaneous Line were exposed to arc welding rays due to the lack of welding screens between themselves and the active welders.

29 CFR 1910.303(b)(1)(i): Electrical equipment was not suitable for installation and use in conformity with the provisions of this subpart and not free from recognized hazards that are likely to cause death or serious physical harm to employees. a) Maintenance Department: On or about April 11, 2023, and at times prior thereto, employees were exposed to electrical shock hazards in that they were allowed to use a job-made extension cord with a steel box enclosure that was intended to be used as a permanent installation.
