29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) Herbicide Batch Room; On or about April 25, 2023, and at times prior, the employer exposed employees to trip hazards, in that the eyewash and body wash station were blocked by exposed hoses, a concrete ledge/curb, a bucket, brooms, and a pipe, while employees batched chemicals. b) Insecticide Batch Room; On or about April 25, 2023, and at times prior, the employer exposed employees to trip hazards, in that the eyewash station was blocked by fixed air compressors, hoses, and a concrete ledge/curb, while employees batched chemicals.

29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Herbicide Batch Room: On or about April 25, 2023 and at times prior, compounders were exposed to eye and skin irritations, in that the employer did not ensure that the emergency eye and body wash stations were not operational, in that tools, brooms, and pipes were being stored in or around the stations blocking access. At the times of the inspection the water to emergency eye and body wash station was turned off. The emergency eye and body wash stations were not kept in a sanitary condition causing the stations to not be in a suitable facility. Not accessible body wash due to concrete ledge/curb for emergency use. b) Insecticide Batch Room: On or about April 25, 2023 and at times prior, compounders were exposed to eye and skin irritations, in that the employer did not ensure that the emergency eye wash station was operational, in that water does not disperse from the eye spouts when the handle was pressed as the intended purpose. Not accessible due to fixed air compressors and concrete ledge/curb blocked access to eye wash station.

29 CFR 1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Herbicide Batch Room; On or about April 25, 2023, and at times prior, the employer exposed employees to health hazards, in that the employer has not provided or enforced medical evaluations in regard to the use of full face elastomeric and Powered Air Purifying (PAPR) respirators for employees who batch chemicals such as but not limited to Synthecol 80%, Diquat 41%, Visor II XRT, and Alkyldimethylbenzyl Ammonium Chloride. b) Insecticide Batch Room; On or about April 25, 2023, and at times prior, the employer exposed employees to health hazards, in that the employer has not provided or enforced medical evaluations in regard to the use of full face elastomeric and Powered Air Purifying (PAPR) respirators for employees who batch chemicals such as but not limited to Synthecol 80%, Diquat 41%, Visor II XRT, and Alkyldimethylbenzyl Ammonium Chloride. The Innova Supply, Inc. dba Solutions Pest & Lawn was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 1515959, citation number 2, item number 1-a and was affirmed as a final order on (date), with respect to a workplace located at 100 Solution Pkwy Thomaston, GA 30286.
