Bonita Springs, FL 34135; On or about April 26, 2023, employees were exposed to walking/work surface hazards in that the pallet attached to the Crown Order Picker (PIT) had multiple boards with damage or nails missing out of the pallet being used as a platform for the lift.

29 CFR 1910.30(a)(3)(iii): The employer did not train each employee in the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses; a) At The Home Depot USA, Store Number 6373, located at 11941 Bonita Beach Rd Se. Bonita Springs, FL 34135; on or about 04/26/2023, the employer exposed employees to fall hazards in that fall protection was not inspected properly when PIT operations were being conducted by employees when retrieving merchandise approximately 6-20 feet in height from the ground using powered industrial trucks. The harness on the Crown Order Picker impact indicator had been exposed indicating that the harness had potentially been involved with a fall and should have been removed from service had employees been properly trained on inspecting fall protection equipment. b.)At The Home Depot USA, Store Number 6373, located at 11941 Bonita Beach Rd Se. Bonita Springs, FL 34135; on or about 04/26/2023, the employer exposed employees to fall hazards in that fall protection was not inspected properly when PIT operations were being conducted by employees when retrieving merchandise approximately 6-20 feet in height from the ground using powered industrial trucks. The body belt on the Ballymore Lift had missing and damaged gramlets exposed indicating that the body belt should have been removed from service had employees been properly trained on inspecting fall protection equipment. c.) At The Home Depot USA, Store Number 6373, located at 11941 Bonita Beach Rd Se. Bonita Springs, FL 34135; on or about 04/26/2023, the employer exposed employees to fall hazards in that fall protection was not inspected properly when PIT operations were being conducted by employees when retrieving merchandise approximately 6-20 feet in height from the ground using powered industrial trucks. The harness on the Raymond Order Picker had fraying and damaged gramlets and should have been removed from service had employees been properly trained on inspecting fall protection equipment.
