No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On May 5, 2023, an employee was exposed to fall hazards of up to 22 feet during roofing operations.   J & K Enterprises of Chicago, Inc. dba J & K Home Improvement & Restoration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1604927, citation number 2, item number 1, and was affirmed as a final order on October 04, 2022, with respect to a workplace located at 11649 W. 195th St, Mokena, IL 60448.  J & K Enterprises of Chicago, Inc. dba J & K Home Improvement & Restoration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11) which was contained in OSHA inspection number 1531912, citation number 1, item number 1, and was affirmed as a final order on August 31, 2021, with respect to a workplace located at 900 Garden Ln., Homewood, IL 60430.  J & K Enterprises of Chicago, Inc. dba J & K Home Improvement & Restoration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1420367, citation number 1, item number 2, and was affirmed as a final order on August 20, 2019, with respect to a workplace located at 611  Wakeman Ave, Wheaton, IL 60187.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
