29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 66 Lyman St. Springfield, MA 01103: On or about May 5, 2023, the employer did not assure that four employees, while working on a commercial roof with unprotected sides and edges more than 11 feet above the next lower level, had been trained in both the nature of fall hazards in the work area and the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems that were used.

29 CFR 1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. 66 Lyman St. Springfield, MA 01103: On or about May 5, 2023, the employer had not prepared a written certification record of fall protection training record which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for four employees who were constructing a new roof on a commercial building and exposed to fall hazards of 11 feet or greater.
