The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.  a. At 20510 East Levy Street Williston, FL 32696; On or about May 9, 2023, the walking and working surfaces of the Wood Chipper Shaving Machines were not maintained in a clean condition, in that, tree log debris was observed spread on the walking platforms, exposing employees to slip and fall hazards.  b. At 20510 East Levy Street Williston, FL 32696; On or about May 9, 2023, the walking and working surfaces of the Pellet Mill Hopper, Pellet Mill Bagger and Pellet Mill Machine #1 & #2 were not maintained in a clean condition, in that, wood dust was observed on the floor and on top of machines electrical components, exposing employees to dust explosion, slip and fall hazards.

At the jobsite, located at  20510 East Levy Street Williston, FL 32696; on or about May 9, 2023, employees were exposed to electric shock hazards, in that secured machine connectors were not utilized for employees operating the Pellet Mill Bagger Machine in close proximity to the open electrical cords that contained exposed and unsecured live electrical wiring while performing pellet bagging operations in the production department.
