29 CFR  1910.147(c)(7)(iv): The employer did not certify that employee training had been accomplished and kept up to date:    a) At 20510 East Levy Street Williston, FL 32696, Log Shaver area, workshop; On or about 09/27/2024, the employer exposed employees to struck-by and caught-in hazards, in that the employer did not ensure a certification record of lockout tagout training was available for all employees who conducted servicing or maintenance on log shaver machine number 1.  b) At 20510 East Levy Street Williston, FL 32696, Log Shaver area, workshop; On or about 09/27/2024, the employer exposed employees to struck-by and caught-in hazards, in that the employer did not ensure a certification record of lockout tagout training was available for all employees who conducted servicing or maintenance on log shaver machine number 2.  c) At 20510 East Levy Street Williston, FL 32696, Log Shaver area, workshop; On or about 09/27/2024, the employer exposed employees to struck-by and caught-in hazards, in that the employer did not ensure a certification record of lockout tagout training was available for all employees who conducted servicing or maintenance on log shaver machine number 3.
