29 CFR 1926.502(d): "Personal fall arrest systems." Personal fall arrest systems and their use shall comply with the provisions set forth below. Effective January 1, 1998, body belts are not acceptable as part of a personal fall arrest system. Note: The use of a body belt in a positioning device system is acceptable and is regulated under paragraph (e) of this section. (a) On or about May 23, 2023, an employee was installing shingles on a second story 6:12 pitch roof of a new residential building, and used a body belt harness, which exposed the employee to a fall hazard of approximately 25 feet and/or other injury to the body in a fall arrest event.

The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. (a) On or about May10, 2023, employees conducting roofing activities (shingle installation) on a second story 6:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was located in the front and centered at the waist (belt area) not located in the center of the wearer's back near shoulder level, or above the wearer's head as required.

(a) On or about May10, 2023, an employee conducting roofing activities (shingle installation) on a second story 6:12 pitch dormer roof was not protected from falling to the lower level. This exposed employee to a fall hazard of approximately Twenty-Five (25) feet on the North side of structure. WI Bilt-Well Roofing, LLC was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13) or its equivalent standard which was contained in OSHA inspection number 1564274, citation number 1, item number 1, and was affirmed as a final order on 3/22/2022, with respect to a workplace located at N90 W27869 S. Red Fox Run Lisbon, WI 53089.
