(a) On or about March 25, 2026, at 681 Hawks Ridge Rd. Grafton, WI 53024, the employer did not ensure employees conducting roofing operations, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

Grafton, WI 53024,  the employer did not ensure that all employees that were installing a roof were trained to recognize fall hazards and how to implement fall protection methods, exposing employees to fall hazards.

(a) On or about March 25, 2026, at 681 Hawks Ridge Rd. Grafton, WI 53024, an employee conducting roofing activities on a 8:12 pitch roof were not properly protected from falling to the lower level. The attachment point of the body harness was front and centered at the chest and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employees to severe spinal and neck injuries.  WI Bilt-Well Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.502 (d)(17), which was contained in OSHA inspection number 1669351, citation number 1, item number 1b, and was affirmed as a final order on 7/18/2023, with respect to a workplace located at W131 S8650  Pintail Ct.
