(a) On or about May 31, 2023, at 896 Kohlwey Dr Grafton, Wisconsin, frequent and regular job site inspections were not conducted to identify and correct safety hazards, including but not limited to fall hazards.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. (a) On or about May 31, 2023, an employee conducting roofing work on an existing residential roof with a 10:12 pitch, was not protected from falling to the lower level. This exposed an employee to a fall hazard of approximately 12 feet on the front side of the house. WI Bilt-Well Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1564274, citation number 1, item number 1 and was affirmed as a final order on March 22, 2022, with respect to a workplace located at N90 W27869 S. Red Fox Run Lisbon, WI 53089.
