29 CFR  1926.701(b):1926.701(b):  All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:  Exterior right side of the building: Rebar that protruded from the fire sprinkler pipe flange was not guarded. Employees were exposed to impalement hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  Ambulance canopy: The ladder used to access the top of the ambulance canopy did not extend at least 3' above the upper landing surface. Employees were exposed to fall hazards.

Employees were exposed to fire and electrical hazards.

29 CFR  1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members such as outlined in 1926.502(b)(2)(i)-(iv) of this section were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  Ambulance canopy: The guardrail system on the right side of the ambulance canopy was missing a midrail. Employees were exposed to fall hazards.
