29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  Location: Shipping and receiving area   On or about 5/17/2023, an employee retrieving rods and materials from an upper storage platform greater than 4 feet above the lower level was not protected from falling.

29 CFR  1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent.  Location: Rod straightening area  a) On or about 6/23/2023, an employee working the rod straightening machine was exposed to a continuous noise level at 65.4% dose of the permissible daily noise exposure, or an equivalent sound level of 87 dbA during the 471-minute sampling period.
