29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about May 16, 2023, during the installation of roofing material at a job site located at Building 6798 Emerson LN, Parma Heights, Ohio 44130; the employer did not ensure employees working on a roof of a multi-unit residential home were protected from a fall of approximately 19.7 feet by ensuring the slack within the employees vertical lifelines was adjusted to limit the fall distance to 6 feet or less.
