Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. On or about August 19, 2025, Located at 5855 Basil Western Road NW, Carroll, Ohio, 43112: The employer failed to ensure that employees were protected from fall hazards while installing roofing shingles on a newly constructed two-story single-family home. As a result, employees were exposed to  fall hazards of approximately 13 feet while working  from the 11/12 steep pitched roof when they were not attached to a lifeline.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about August 19, 2025, Located at 5855 Basil Western Road NW, Carroll, Ohio, 43112: The employer failed to ensure the employees were wearing eye protection while operating pneumatic nail guns to install roof materials on a two story new build single family home.  Sugarvalley Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard  29 CFR  1926.102(a)(1), which was contained in OSHA inspection number 1670928, citation number 1, item number 1 and was affirmed as a final order on August 16, 2024, with respect to a workplace located at Building 6798 Emerson LN, Parma Heights, Ohio 44130.

29 CFR  1926.502(d)(16):Personal fall arrest systems, when stopping a fall, shall:  29 CFR  1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level.  a. On or about August 19, 2025, Located at 5855 Basil Western Road NW, Carroll, Ohio, 43112: The employer failed to ensure that employees were protected from fall hazards while installing roofing shingles on a newly constructed two-story single-family home. As a result, employees were exposed to  fall hazards of over 6 feet, and up to approximately 13 feet, while working  from the roof with personal fall harnesses attached to rope lifelines that were not rigged to prevent a free fall of 6 feet or less.   Sugarvalley Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR  1926.502(d)(16)(iii), which was contained in OSHA inspection number 1670928, citation number 1, item number 2 and was affirmed as a final order on August 16, 2024, with respect to a workplace located at Building 6798 Emerson LN, Parma Heights, Ohio 44130.
