On or about August 19, 2025, Located at 5855 Basil Western Road NW, Carroll, Ohio, 43112: The employer failed to ensure that employees were protected from fall hazards while installing roofing shingles on a newly constructed two-story single-family home. As a result, employees were exposed to fall hazards of approximately 13 feet while working from the 11/12 steep pitched roof when they were not attached to a lifeline.

Sugarvalley Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1670928, citation number 1, item number 1 and was affirmed as a final order on August 16, 2024, with respect to a workplace located at Building 6798 Emerson LN, Parma Heights, Ohio 44130.

As a result, employees were exposed to fall hazards of over 6 feet, and up to approximately 13 feet, while working from the roof with personal fall harnesses attached to rope lifelines that were not rigged to prevent a free fall of 6 feet or less. Sugarvalley Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.502(d)(16)(iii), which was contained in OSHA inspection number 1670928, citation number 1, item number 2 and was affirmed as a final order on August 16, 2024, with respect to a workplace located at Building 6798 Emerson LN, Parma Heights, Ohio 44130.
