29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation:   On or about April 21, 2026, the employer failed to ensure the employees were wearing eye protection while operating pneumatic nail guns to install roofing materials on a two story new build single family home.  Sugarvalley Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard  29 CFR  1926.102(a)(1), which was contained in OSHA inspection number 1844640, citation number 2, item number 1 and was affirmed as a final order on December 4, 2025, with respect to a workplace located at 5855 Basil Western Road NW, Carroll, Ohio, 43112  Sugarvalley Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard  29 CFR  1926.102(a)(1), which was contained in OSHA inspection number 1670928, citation number 1, item number 1 and was affirmed as a final order on August 16, 2024, with respect to a workplace located at Building 6798 Emerson LN, Parma Heights, Ohio 44130.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 21, 2026, during the installation of roofing material on a two story new build single family home located at 1914 Lynn Road, Kent OH 44240, the employer did not ensure employees working on the roof were protected from a fall of approximately 12 - 20 feet and greater, by utilizing a fall protection system.  Sugarvalley Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard  29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1844640, citation number 1, item number 2 and was affirmed as a final order on December 4, 2025, with respect to a workplace located at 5855 Basil Western Road NW, Carroll, Ohio, 43112
