29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: On or about July 11, 2023, and at times prior thereto, employee(s) were observed having facial hair and using a head balaclava while wearing a required tight-fitting respirator. Thus, exposing employees to physiological effects, unforeseen medical problems, and respiratory illnesses in the following instances (a-b): a) On May 25, 2023, an employee had facial hair and used a head balaclava while wearing a tight-fitting respirator when working in the 12-Mesh building. b) On July 11, 2023, an employee had facial hair and used a head balaclava while wearing a tight-fitting respirator when working in the 12-Mesh building.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of particulates not otherwise regulated (PNOR)Total Dust listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 15 mg/m3: On or about July 11, 2023, and at times prior thereto, in the 12-Mesh building, in an 8-hour time-weighted average (TWA), employee(s) were exposed to particulates not otherwise regulated (PNOR)-total dust at concentrations of approximately 25 micrograms per cubic meter of air, which is approximately one-half (1.5) times the permissible exposure limit (PEL). Exposure was derived from a sample collected over 412 minutes, and zero valve exposure was assumed from a 68-minute non-sampled period.
