This exposed the employee to a fall hazard of approximately 10 to the ground below.  (b) On or about June 1, 2023, employees were exposed to a fall hazard while using an unsafe means to access and egress the top of a decorative pergola at approximately 10 feet above the ground level.  Employees used a forklift with a wood pallet placed on the forks of the forklift and were elevated and lowered while standing on the wood pallet to access and egress the top of the pergola.
