29 CFR 1904.41(a)(1)(i): The employer's establishment had 20-249 employees during the previous calendar year and is classified in an industry listed in Appendix A to Subpart E of Part 1904, but the employer failed to electronically submit information from the establishment's OSHA Form 300A by March 2nd. (a) On or about March 2, 2026, at the establishment located at 4838 Douglas Avenue, Racine, WI 53402, the employer did not electronically submit the required OSHA Form 300A Annual Summary information for calendar year 2025 to OSHA's Injury Tracking Application (ITA), as required by 29 CFR 1904.41(a)(1)(i). Milaeger's Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.41(a)(2), which was contained in OSHA inspection number 1675915, citation number 1, item number 1 and was affirmed as a final order on August 18, 2023, with respect to a workplace located at 4838 Douglas Ave., Racine, WI.
