29 CFR 1926.451(b)(1): Each platform on all working levels was not fully planked or decked between front uprights and the guardrail supports:  a) On June 7, 2023, employees working from tubular welded frame scaffold platform were exposed to fall hazards of more than approximately 24 feet where the scaffold platform was not fully decked.   Joseph Birkenheier Tuckpointing and Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1420275 citation number 1, item number 1 and was affirmed as a final order on July 17, 2020, with respect to a workplace located at 311 W Cossit Ave., La Grange, IL 60053.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On June 7, 2023, employees were exposed to fall hazards of approximately 12.8 feet while performing masonry work from the roof of a residential structure.   Joseph Birkenheier Tuckpointing and Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1420275 citation number 1, item number 1 and was affirmed as a final order on July 17, 2020, with respect to a workplace located at 311 W Cossit Ave., La Grange, IL 60053.
