Occurred  on or about 06/07/2023.  b) Innovative Metals Inc., - Rollformer area: The point of operation on the roll-former machines, including, but not limited to, LOCKFORMER, MODEL# GEM 3300, S/N SP1182, LOCKFORMER (GREEN), LOCKFORMER (BLUE) Model # 2916, and ROLLFORMER -SEAM PANEL LINE #17 were not guarded to protect employees against pinch points from the point of operation. One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   a) Innovative Metals Inc., - Rollformer area: Employees were exposed to multiple unguarded pinch points s in the roll-former machines, including, but not limited to, LOCKFORMER, MODEL# GEM 3300, S/N SP1182, LOCKFORMER FLUSH LOCK (GREEN), LOCKFORMER BATHEN TILE(BLUE) Model # 2916, and the two half profile ROLLFORMER machines (Zip and Tapering mill).
