29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation  (a) On or about June 22, 2023, employees were operating pneumatic nail guns without eye protection while installing roof shingles, exposing employees to a struck by hazard and eye injury.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   (a) On or about June 22, 2023, the employer did not train employees to be authorized operators of the JGL Lift, presenting a fall hazard to employees in the basket and crushing hazards to other employees.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:  (a) On or about June 22, 2023, the employer did not ensure the employee working from the JLG aerial lift was using fall protection while performing framing work. This exposed the employee to a 15-foot fall hazard to the ground below.
