29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Location: 622 Bloomfield Ave., Bloomfield, NJ 07003  a) The employer failed to instruct employees engaged in residential construction activities  using a JLG,  Model 1350SJP , Ultra Series, Telescopic Boom Lift on the lanyard attachment requirements as described in the machine's operator's manual.  Two employees had their lanyards attached to a single anchorage point while working from the aerial lift.  Violation was observed on or about June 29, 2023

29 CFR  1926.453(b)(2)(i): Lift controls were not tested each day prior to use to determine that such controls are in safe working condition.  Location: 622 Bloomfield Ave, Bloomfield, NJ 07003  a) Employer did not ensure a pre-use inspection was completed on a JLG,  Model 1350SJP , Ultra Series, Telescopic Boom Lift to ensure the safe working conditions of the controls. Employees were operating the equipment with a malfunctioning override switch.   Violation was observed on or about June 29, 2023.
