29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Side Elevation of Building - On or about June 29, 2023, employees were exposed to the hazard of falling approximately 25 feet while working from ladder jack scaffolding without the benefit of fall protection.  b) Front Elevation of Building - On or about June 29, 2023, employees were exposed to the hazard of falling approximately 17 feet while working from ladder jack scaffolding without the benefit of fall protection.  No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) Side Elevation of Building - On or about June 29, 2023, employees were exposed to the hazard of falling approximately 25 feet while working without fall protection from pitched roof surfaces.  b) Front Elevation of Building - On or about June 29, 2023, employees were exposed to the hazard of falling approximately 14 feet while working without fall protection from pitched roof surfaces.  JM Superior Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1506755, citation 1, item 1, and was affirmed as a final order on January 17, 2022, with respect to a worksite located at 1492 Sheridan Road, Highland Park, Illinois.   JM Superior Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1459185, citation 1, item 1, and was affirmed as a final order on February 20, 2020, with respect to a worksite located at 125 Woodstock Ave., Kenilworth, Illinois.   No abatement certification or documentation required for this violation.
