29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  Employees engaged in servicing and maintenance activities at the Oriental Trading Company, Inc. located at  4206 South 108th Street, Omaha, Nebraska, were exposed to caught-in and caught-on hazards in that the employer failed to ensure lockout/tagout procedures for the 3M-Matic Case Sealing System included specific: i. Statement of the intended use of the procedures, ii. Procedural steps for shutting down, isolating, blocking, and securing the machine to control hazardous energy. iii. Procedural steps for placement, removal or transfer of the lockout devices, such as type of device and location, and iv. Requirements for testing machine to determine or verify the effectiveness of lockout/tagout devices, such as location of raising switch.

The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection:  Employees engaged in maintenance activities at the Oriental Trading Company Located at 4206 South 108th Street, Omaha, Nebraska, were exposed to caught-in and caught-on hazards in that the employer failed to conduct periodic inspections of the energy control procedures for the 3M-Matic Case Sealing Systems.

29 CFR  1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets:  Employees engaged in forklift activities at the Oriental Trading Company located at 4206 South 108th Street and 11112 I Street, Omaha, Nebraska, were exposed to chemical contact in that employees were not provided training associated with working with, near, and/or around acid batteries or other hazardous chemicals specific to the work area.
