On or about December 19, 2023, employees engaged in commercial manufacturing activities on the production floor of the facility located at 4206 South 108th Street, OMAHA, NE 68137, were exposed to amputation hazards in that employees were conducting maintenance activities on the HSD AT/MT 1073-120 Serial Number 2019038938 machine (COLEX 3) without fixing a lockout and/or tagout device as means of ensuring the isolation of the hazardous energy sources.

29 CFR 1910.147(f)(1)(ii): 29 CFR 1910.147(f)(1)(ii): Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; On or about December 19, 2023, employees engaged in commercial manufacturing activities on the production floor of the facility located at 4206 South 108th Street, OMAHA, NE 68137, were exposed to amputation hazards in that the employees were not removed from the hazardous areas of the HSD AT/MT 1073-120 Serial Number 2019038938 machine (COLEX 3) before conducting maintenance activities. The result of this was an employee having the tip of an index finger amputated.
