29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Employees were exposed to amputation hazards. On or about June 27, 2023, employees engaged in fabrication activities in the fabrication shop at the RAS machine with serial number 12778 were exposed to amputation hazards in that no point of operation guards were present that would prevent employees from entering parts of the body within the operating cycle of the machine.
