29 CFR 1910.213(c)(2): Hand-fed circular ripsaw(s) were not furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator: a) Shed Production Building: On or about January 27, 2025, employees required to use the Grizzly 10" table saw were exposed to kick-back and amputation hazards during rip cutting as this saw was operated without a spreader device. This condition exposed employees to struck-by and amputation hazards.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Shed Production Building: On or about January 27, 2025, employees were required to wear tight-fitting, half-face respirators during water-based paint spraying operations. The employer did not implement all required elements of a Respiratory Protection Program that would include, but is not limited to: (1) medical evaluations for employees required to wear tight-fitting, half-face respirators, (2) fit testing for employees required to wear tight-fitting, half-face respirators, (3) training for employees regarding the hazards they may encounter during the required use of tight-fitting, half-face respirators, (4) training for employees regarding the limitations of tight-fitting, half-face respirators they are required to use, and (5) procedures for evaluating the effectiveness of the Respiratory Protection Program. This exposed employees to potential pulmonary and cardiovascular hazards due to the use of respirators.
