Location: 17 Stanton Street, Malden, MA; On or about 07/11/2023, employees were allowed to utilize temporary roof anchors which were not installed per manufacturer instructions to support 5,000lbs per employee. a) Anchorage point on mid section of residential roof near chimney was not secured per manufacturer instructions and had multiple missing fasteners. b) Anchorage point on the front section of residential roof nearest the street/front of home was not secured per the manufacturer instructions and had multiple missing fasteners.
