Location: Job site;  On or about 09/19/2024, employees were allowed to utilize pneumatic nail guns without proper eye/face protection to protect against potential flying objects.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.  Location: Job site;  On or about 09/19/2024, employees were allowed to utilize personal fall arrest system lifelines which were rigged in a manner that created excessive slack in the line and would allow the affected employees to fall more than 6 feet to a lower level.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Location: Job site;  On or about 09/19/2024, employees were allowed to engage in roofing activities without the use of fall protection on a residential roof that was 14-feet (rear of residence) above the ground below.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee.  Location: Job site;  On or about 09/19/2024, employees were allowed to utilize temporary roof anchors which were not installed per manufacturer instructions to support 5,000lbs per employee.   a) Anchorage points along the peak of residential roof from nearest the chimney to the end of the residence furthest from the driveway were not secured per manufacturer instructions and had multiple missing fasteners and were separating from the roof.  Greater Boston Roofing Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.502(d)(15)), which was contained in OSHA inspection number 1682103, citation number 1, item number 1-a and was affirmed as a final order on 05/12/2024, with respect to a workplace located at 17 Stanton Street, Malden, MA 02148.  Greater Boston Roofing Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.502(d)(15)), which was contained in OSHA inspection number 1622620, citation number 1, item number 1-b and was affirmed as a final order on 03/10/2023, with respect to a workplace located at 109 Chase Road, Marlborough, MA 01752.
