29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about July 7, 2023, in the bacon slicing and packing area, employee(s) engaged in sanitizing activities on the North-Slice-Anco Slicer Ln-1 Transfer Conveyor were exposed to caught-in and amputation hazards in that procedures for the control of potentially hazardous energy were not utilized. Employee amputated his pinky finger and part of his hand.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed: In the bacon slicing and packing area, employee(s) engaged in sanitizing activities on the North-Slice-Anco Slicer Ln-1 Transfer Conveyor were exposed to amputation, caught in, and lacerations hazards in that the employer did not certified that periodic inspections of the existing energy control procedures have been conducted. Certification must include the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.
