29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 1073 Wood Duck Street, Fruitland, ID 83619: On or about July 18, 2023, employees were exposed to falls of approximately 9 feet up to 18 feet when performing roofing activities on a multi-level home with approximate 6:12 sloped roof without properly using fall protection. Ropes with rope grab devices were used with the devices placed so that workers would fall more than six feet and /or strike the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) A fiberglass extension ladder was used to access the lower roof level with the second rung from the top even with the eave. The side rails extended approximately 15 to 20 inches above the sloped roof being accessed.
