On or about July 17, 2023, during the installation of roofing material at the job site located at Lot 53 at Retreat at Rosemont, 3455 Birch Lane, Fairlawn Ohio 44333; the employer did not ensure employees working on the roof of a new construction residential structure were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
