29 CFR 1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about March 19, 2025, during the installation of roofing material at the job site located at 2107 Via Luna Cir, Massillon Ohio 44646; the employer did not ensure employees working on a residential structure were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material with air driven nail guns. Pine Grove Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1) Eye and face protection), which was contained in OSHA inspection number 1684023, citation number 01, item number 01 and was affirmed as a final order on (10/11/2023), with respect to a workplace located at Retreat at Rosemont Lot 53, 3455 Birch LN, Fairlawn Ohio 44333.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 19, 2025, during the installation of roofing material at the job site located at 2107 Via Luna LN, Massillon Ohio 44646; the employer did not ensure employees working on a roof of a residential structure were protected from a fall height approximately 9.4 feet above the ground by utilizing a fall protection system. Pine Grove Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use fall protection), which was contained in OSHA inspection number 1684023, citation number 01, item number 02 and was affirmed as a final order on (10/11/2023), with respect to a workplace located at Retreat at Rosemont Lot 53, 3455 Birch LN, Fairlawn Ohio 44333.
