29 CFR 1915.71(d)(4):Posts shall be fitted with bases supported on a firm foundation to distribute the load. When wooden sills are used, the bases shall be fastened thereto. (a) USNS Montford Point, Pump room lower deck, 5555 North Channel Avenue, Portland, Oregon 97217, on or about July 24, 2023 and at times prior thereto, employees were exposed to fall hazard from scaffold assemble without fitted with bases supported on a firm foundation to distribute the load and fastened to the wooden sill bases. Resulting in potential scaffold failure which would cause serious injuries, disability and death.
