OSHA Instruction: Shipyard Employment "Tool Bag" Directive - CPL 02-00-162 allows for the applicability of 29 CFR 1910.212.(a)(3)(ii) in the ship building and ship repair industry.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. (a) Tool Shop Third Floor - On or about February 10, 2025 and at times prior thereto, the employees were exposed to electrical hazards that would result in cardiac arrest, severe burns and even death from contact with live electrical conductors rated at 60 AMP and 600 Volts Alternating Current (VAC) through the unused openings on General Electric (GE) Heavy Duty Safety Switch. OSHA Instruction: Shipyard Employment "Tool Bag" Directive - CPL 02-00-162 allows for the applicability of 29 CFR 1910.305(b)(1)(ii) in the ship building and ship repair industry.

29 CFR 1915.56(b)(2):Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected. M/V Laconte - Starboard Fuel Oil Settling Tank- On or about February 10, 2025 and at times prior thereto, employees performing welding activities were exposed to electrical shock in that they were welding using welding cables that had been damaged and then repaired with electrical tape, approximately 32 inches from the lead.

29 CFR 1915.73(c):When employees are working around open hatches not protected by coamings to a height of 24 inches or around other large openings, the edge of the opening shall be guarded in the working area to height of 36 to 42 inches, except where the use of such guards is made impracticable by the work actually in progress. M/V Laconte - Car Deck - On or about February 10, 2025 and at times prior thereto, employees performing ship repair activities were exposed to fall hazards in that they were working near an open hatch with a coaming height of 10 inches that was not guarded to prevent falls.
