29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:   a- 903 High Mountain Road: An employee engaged in framing activities from the second section of a tubular welded frame scaffold work platform was exposed to fall hazards of approximately 14ft to the ground below in which 1-2 planks were used as a scaffold work platform as full planking of the scaffold work platform was not utilized. Condition observed on or about 07/25/2023.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a- 903 High Mountain Road: An employee engaged in framing activities from the second section of a tubular welded frame scaffold work platform was exposed to fall hazards of approximately 14ft to the ground below in which a means of scaffold access was not utilized. The employee climbed on the scaffold frame to the lower roof. Condition observed on or about 07/25/2023.

Adrian Construction Group LLC does not ensure that employees use appropriate eye protection while engaged in residential construction activities. This violation was most recently observed, on or about 07/25/2023, at 903 High Mountain Road, Franklin Lakes, NJ. The employees were engaged in framing activities and positioned on a roof using a pneumatic nail gun are exposed to eye loss and eye injuries as eye protection which was readily available for use onsite was not being used.   Pursuant to 29 CFR 1903.19, within 1 day, the employer must submit documents describing the steps it is taking to ensure compliance, including a description of the type of eye protection provided and the steps taken to enforce use of appropriate eye protection where employees are exposed to possible danger of fall hazards.
