29 CFR 1904.41(a)(2): The establishment had 20 or more but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified as an industry listed in appendix A to subpart E of this part, but did not electronically submit information from OSHA 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form: a) BR Pallet, Inc. located at 21395 Hancock Ct. Rd. 7, Alvada, Ohio, 44802: On or about March 2, 2023, the employer failed to submit the company's OSHA Form 300A Summary of Work-Related Injuries and Illnesses for the 2022 calendar year to OSHA's Injury Tracking Application (ITA). The employer had an average of 20 employees at the worksite requiring them to submit the OSHA 300A information to OSHA by March 2, 2023. Failure to keep updated logs and providing the documents to OSHA could inhibit the employer from identifying trends related to occupational injuries or illnesses at their workplace.
