29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards: a) On the roadway at the intersection of E. Cochran Street and S. Catherine Street, in Middletown, DE 19709: On or about July 26, 2023, employees were exposed to hazards onsite including but not limited to struck-by and/or caught-in hazards when they performed flagger duties at an intersection to stop traffic and assist vehicles, such as but not limited to construction vehicles entering the roadway closure. The employee did not have a sign posted at the point of hazard from all directions of travel to indicate that there would be a flagging operation or the need for motorists or construction traffic to stop for vehicles entering exiting the work zone closure. b) On the roadway at the intersection of E. Redding Street and S. Catherine Street, in Middletown, DE 19709: On or about July 26, 2023, employees were exposed to hazards onsite including but not limited to struck-by and/or caught-in hazards when they performed flagger duties at an intersection to stop traffic and assist vehicles, such as but not limited to construction vehicles entering the roadway closure. Catherine Street, in Middletown, DE 19709: On or about July 26, 2023, Employees were exposed to multiple hazards onsite including but not limited to struck by/caught in hazards when they performed site preparation work for the replacement and upgrade to ADA sidewalks, when signs were not posted and the roadway was closed in both directions for equipment and material as well as trucks entering/entering to deliver additional materials. b) Sidewalk replacement at the roadway at the intersection of E. Redding Street and S. Catherine Street, in Middletown, DE 19709: On or about July 26, 2023, Employees were exposed to multiple hazards onsite including but not limited to struck by/caught in hazards as they performed site preparation work for the replacement and upgrade to ADA sidewalks, when signs were not posted and the roadway was closed in both directions for equipment and material as well as trucks entering/entering to deliver additional materials.

Redding Street, in Middletown, DE 19709: On or about July 26, 2023, the employer did not install any type of protections to prevent the impalement hazards associated with the reinforcing steel protruding from the ground where employees walked across to access the work area.
