Located at 300 S Jefferson Ave, Plain City, OH 43064:  On or about August 9, 2023, employees placed an unsecured extension ladder measuring approximately 14' on a block of soft foam and used the ladder to access/egress a commercial roofing project at a height of approximately 12'.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.  a.  Located at 300 S Jefferson Ave, Plain City, OH 43064:  On or about August 9, 2023, employees used an approximately 14' extension ladder with broken feet to access and egress a commercial roofing project approximately 12' from the ground.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  a.  Located at 300 S Jefferson Ave, Plain City, OH 43064:  On or about August 9, 2023, the employer failed to protect employees engaged in removing and attaching shingles to a low-slope roof from falling off of unprotected sides and edges, ranging from approximately 12' to 18', by fall prevention or protection equipment and/or systems.   Jara Construction, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 1623338, citation number 1, item number 1 and was affirmed as a final order on December 12, 2022, with respect to a workplace located at 6015 Peck Lane, New Albany, OH 43054  Jara Construction, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 1608786, citation number 1, item number 1 and was affirmed as a final order on September 21, 2022, with respect to a workplace located at 2728 Cranbrooke Lane, Delaware, OH 43015  Jara Construction, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 1593906, citation number 1, item number 1 and was affirmed as a final order on October 26, 2022, with respect to a workplace located at 6591 Littlefield Drive, Westerville, OH 43081.
