(a) Dock, 15th Street, Huntington WV: On June 29, 2023, the crane operator maneuvered a draft of I-beams in proximity to an employee who was working within a barge. The crane operator did not have a visual of the draft or a signal from someone observing the load.

(a) Dock, 15th Street, Huntington WV: On June 29, 2023, during a barge loading, an employee was hospitalized after being struck by and caught between the draft being moved by a crane and the barge wall. The work-related incident was not reported to OSHA by the company within 8 hours.
