29 CFR  1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  a.) On or about August 2, 2023, the employer did not ensure employees lancing the shelled over pour spout of the 65 ton furnace containing molten metal wore a properly fitting Steiner flame retardant cotton jacket to protect the body and upper extremities; and wore a face shield to protect the face.  Molten metal ejected from the pour spout causing a burn to an employee's abdomen.  b.) On or about July 25, 2023, the employer did not ensure personal protective Steiner flame retardant cotton jackets and pants worn by employees who were exposed to molten metal on the melt deck were maintained in a reliable condition in that, the employer did not document the number of washes to ensure that the manufacturer's recommended number of home washes was not exceeded so that the flame retardancy was maintained.
