29 CFR  1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work:    a-107 E Prospect Street, Waldwick NJ: (Roof) Employees were exposed to electrical shock hazards when performing roofing work near  two (2) 120 volt energized powerline. Condition observed on or about 08/14/2023.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) 107 E Prospect Street, Waldwick, NJ/Roof:  Employees were engaged in roofing activities using pneumatic nail guns while installing new roof shingles were not protected from eye injury hazards. The employees were not using any means of eye protection which had not been brought to the work site, on or about 08/14/2023.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 107 E Prospect Street, Waldwick, NJ/Roof:  Employees were engaged in roofing activities and positioned on a 6 on 12 pitch roof and were not protected from fall hazards of approximately 10 ft to the asphalt below. The employees were not using any means of a fall protection system which was readily available for use onsite, on or about 08/14/2023.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.     a-107 E Prospect Street, Waldwick NJ: (Lower Flat Roof) Employees were exposed to fall hazards of approximately 10ft while accessing a lower roof using a portable extension ladder whose ladder side rails only extended +/- 1 foot and did not extended 3ft above the upper landing surface. Condition observed on or about 08/14/2023.
